Sue ChaffmanSue Chaffman offers an extensive background in medical care management, rehabilitation, and discharge planning. Sue is passionate about elder-care and was one of the first graduates in Aging Studies with a M.A. in Gerontology(with honors) from the University of South Florida. She has a B.A. in Psychology from Auburn University. Sue has over 20 years experience as a geriatric and rehabilitation care manager. In addition to in-home/field services, Sue has worked in hospital, outpatient/ rehabilitation, and sub-acute settings. Her experience includes medical social work for a geriatric hospital in New Zealand. She has expertise in disability management of clients with stroke, head injuries, and those with chronic, progressive conditions.

Sue enjoys her work as a care manager to help “those who cared for us.” “Assisting frail, elderly or vulnerable persons should include helping them to understand options as they navigate difficult life experiences and choices.”

Sue excels at problem solving and liaises with other specialists/caring professionals to address unique client/family situations and medically complex cases. She responds when familieSue Chaffman West Florida Care Managements require urgent or after-hours services. She assists families who may disagree regarding care decisions.

Professional qualifications include: Certified Medical Case Manager (CCM), Certified Rehabilitation Counselor (CRC) and memberships in the National Association of Professional Geriatric Care Managers (since 1998) and the Florida Geriatric Care Management Association.


Sue is a long-time Pinellas County (Seminole, FL) resident and is married with two children, one of whom is serving in the U.S. Air Force. Sue is an avid cyclist and enjoys traveling.
